Sentra Care are working in partnership with our client, a recognised leader in the Community Complex Care sector , to recruit an experienced Clinical Lead to support adults and children with complex healthcare needs in the community.
This is a highly autonomous, clinically focused role offering a true hybrid working model, with the opportunity to make a meaningful impact on client outcomes while maintaining a healthy work–life balance.
The Role
you will be responsible for managing a community-based caseload across Reading and Oxford , ensuring the delivery of safe, high-quality, nurse-led care.
The role will consist of:
3 days per week in the community , completing client assessments, staff training, touchpoints, reviews, and clinical oversight
2 days per week working from home for care planning, governance, and MDT coordination
