What you?ll do Lead support planning, risk management and deliver person-centred support to patients who are admitted to Mental Health wards. Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads

Ensuring information is logged and recorded accurately on customers support files and systems we use

Support Customers with housing, social and financial goals to ensure discharge from hospital is timely.

Attend ward meetings relating to customers to ensure communication is efficient.

Liaise with TEWV teams and ward staff, and other supporting providers to ensure that support is holistic and appropriate to reduce risk of delayed discharges and readmission.

You have Passion and experience in supporting others ideally in a supported housing setting

Level 3 Diploma in Care or equivalent experience (or are willing to work towards it)

Experience assessing referrals and coordinating support

The ability to work under your own initiative, remain calm under pressure and have a resilient approach

The practical bits Working from West Park Hospital in Darlington primarily, with an expectation if required to cover Lanchester Road hospital, Durham and our Post Discharge offer across the Co Durham area

Working hours between 8:30am and 4:30pm

You need an Enhanced DBS with barring list (we pay for that)

A vehicle insured for business use (and a full UK driving license!), we?ll pay for any business mileage
