Mental Health Practitioner

Deliver high-quality, person-centred care to adults with complex mental health needs in the community.
Work collaboratively within a multidisciplinary team to develop and implement care plans.
Conduct assessments, risk evaluations, and reviews in line with clinical governance and safeguarding protocols.
Provide therapeutic interventions and recovery-focused support.
Liaise with service users, carers, and external agencies to ensure continuity of care.
Maintain accurate and timely clinical documentation.
Participate in team meetings, supervision, and ongoing professional development.

Exeter City Core Community Mental Health Team (CMHT): Full-Time, 37.5 hours/week, with flexible and hybrid working options. We are seeking enthusiastic and compassionate Band 5 Registered Mental Health Nurses (RMNs), Occupational Therapists (OTs), or Social Workers to join our innovative and supportive team. Preceptee applicants are also welcome for a nurturing community mental health practice environment.
