We are a specialist complex care service supporting adults (18+) living with severe and enduring mental health needs, often alongside substance misuse or other dual diagnoses. You will be part of a small, values-driven team that believes in genuine recovery, independence and dignity - not just 'managing' risk.

What you'll be doing

• Providing one-to-one, holistic support to adults with complex mental health presentations and dual diagnosis i

• Using de-escalation, relational practice and clear boundaries to keep people safe while promoting choice and control.?

• Supporting people with daily living skills (medication support, appointments, budgeting, routines, community access) in line with person-centred care and recovery plans.

• Working closely with multidisciplinary teams, families and other agencies to ensure consistent, joined-up care.

• Accurately recording notes, monitoring risk and contributing to reviews, always in line with CQC standards and safeguarding duties.

• Work with a smaller caseload so you can build real therapeutic relationships, not just 'cover visits'.

• Be part of a specialist complex mental health and dual diagnosis pathway where your skills are genuinely valued and developed.

• Access ongoing training around trauma-informed care, substance misuse, positive behaviour support and relapse prevention.

• Support people at their most vulnerable - stepping in where inpatient admission may otherwise be needed, and helping them stay safely in the community.

You must have:

• Minimum 1 year paid experience supporting adults with complex mental health needs (e.g. psychosis, personality disorder, severe mood disorders).

• Experience supporting people with dual diagnosis (mental health and substance misuse) or a clear understanding and willingness to work in this area.

• Confidence in lone working, using risk management plans and following safeguarding procedures.

• Strong communication skills, emotional resilience and a genuinely non-judgemental, recovery-focused approach. xbpsjku

• Health & Social Care qualification (NVQ/Diploma Level 2 or 3, or equivalent).
