Salary: £40,000 £45,000 DOE + Mileage (45p/mile)
Hours: MondayFriday | 9am5pm | Hybrid & Flexible
Contract: Permanent | Full-Time

As a Nurse Case Manager RGN, youll coordinate complex-care packages for adults and children, using your clinical expertise to assess, plan, and manage person-centred care in home and community settings. This hybrid position offers regional travel, autonomy, and genuine career progression.
Key Responsibilities
  • Manage a caseload of community complex-care clients across the Coventry region, ensuring safe and effective delivery.
  • Carry out clinical assessments, risk reviews, and care-plan updates in line with best practice and CQC standards.
  • Provide clinical supervision, mentorship, and competency sign-offs for carers and support staff.
  • Conduct clinical audits, medication checks, and quality reviews to maintain compliance and governance.
  • Liaise with families, ICBs, case managers, and MDT professionals to coordinate holistic care.
  • Deliver training and clinical support to teams during both routine and emergency situations.
  • Drive continuous improvement and uphold professional nursing standards as a Nurse Case Manager RGN.

Requirements
  • Registered Nurse (Adult RGN) with valid NMC registration essential
  • Background in complex care, community nursing, or case management
  • Strong skills in clinical assessment, risk management, and documentation
  • Excellent leadership and communication skills
  • Full UK driving licence and access to a vehicle essential
