Work collaboratively with NHS partners, commissioners and the local authority to strengthen integrated care pathways.
Manage service performance, quality standards, workforce development and budgets.
This role offers a unique opportunity to shape the future of intermediate care services and play a vital role in improving system flow and patient outcomes across the region.
