Senior Care Assistant / Team Leader | Bristol & South West

The Senior Care Assistant / Team Leader plays a vital role in leading high-quality care delivery within residential, supported living or domiciliary services. Responsible for overseeing daily care routines, supervising care staff, administering or overseeing medication, and ensuring care plans are followed accurately, the post holder acts as a key support to management and a role model for the wider care team.
The role requires an experienced, confident and compassionate care professional with the ability to lead by example, maintain compliance with care standards, coordinate staff on shift, and ensure the wellbeing, safety and dignity of individuals receiving support. Strong communication, organisational skills and the ability to remain calm under pressure are essential.
Key Responsibilities
1. Care Delivery & Service Quality
Provide high-quality, person-centred care in line with individual care plans.
Lead by example, demonstrating best practice in personal care, communication and safeguarding.
Support individuals with daily living tasks, mobility, nutrition, continence, medication, and community engagement.
Promote independence, choice, and dignity at all times.
2. Staff Supervision & Shift Leadership
Lead shifts and allocate tasks to care staff to ensure safe, effective staffing levels.
Provide guidance, support and mentoring to junior care workers.
Conduct spot checks, handovers and maintain effective communication across shifts.
Support new staff through shadowing, training and induction processes.
3. Medication Management
Administer or oversee the administration of medication in line with organisational policy (where trained and competent).
Maintain accurate MAR charts and escalate medication issues promptly.
Conduct medication audits as required.
4. Care Planning & Documentation
Ensure care plans, risk assessments and support documents are followed and kept up to date.
Record all care notes, incidents and communications accurately and professionally.
Report changes in individuals needs to senior management promptly.
5. Safeguarding, Risk & Health & Safety
Identify and report safeguarding concerns in line with policy.
Support safe working practices including infection control, risk management and health & safety compliance.
Respond to emergencies or incidents, ensuring prompt escalation if required.
Promote safe use of equipment and ensure staff adhere to moving & handling protocols.
6. Communication & Professional Relationships
Build positive relationships with service users, families, professionals and colleagues.
Attend team meetings and contribute to service improvements.
Liaise with visiting professionals including nurses, social workers and therapists.
Person Specification
Essential
Minimum 12 years experience in a care or support role.
Experience leading shifts or mentoring junior care staff.
Strong understanding of person-centred care and safeguarding.
Confident with care documentation and communication.
Willingness to complete senior-level training (medication, leadership, etc.).
Compassionate, reliable and professional approach.
Desirable
NVQ/QCF Level 2 in Health & Social Care (minimum); Level 3 preferred.
Medication administration training/experience.
Experience in residential care, supported living, domiciliary care or extra care services.
Knowledge of CQC requirements and auditing.
Car driver (desirable for community-based roles).

Working Environment
This role may involve working in residential care homes, supported living settings, extra care schemes or community-based services across Bristol and the South West. The post includes shift work such as early mornings, evenings, weekends and possible sleep-ins or night shifts depending on service requirements. The role can be physically demanding and requires the post holder to be comfortable supporting individuals with varying levels of need.
