Key Responsibilities
Develop and maintain personalised support plans aligned with LA Care standards. Keep them updated to reflect current needs and goals.
Deliver key work sessions in line with support plan goals through one-to-one education and reflective conversations.
Provide tools, guidance, and referrals to external agencies to help young people achieve their goals.
Work with young people to identify education and career aspirations, connecting them with appropriate resources.
Maintain accurate records of progress, behaviours, and interactions, adhering to confidentiality and reporting requirements within the outlined timeframe.
Complete daily checklists, cleaning tasks, and review communication logs.
Safely manage the running of the home, ensuring all allocated tasks are completed.
Maintain cleanliness, presentation, safety, and security of the home to regulatory standards.
Take responsibility for petty cash, ensuring correct usage and accurate recording.
Support planning, arranging, and monitoring of appointments for young people.
Review and update risk assessments regularly, reflecting the young persons needs and mitigating potential hazards.
Adhere to Lone Worker Policy and protocols to ensure staff safety.
Respond calmly and effectively to crises or emergencies using appropriate de-escalation techniques.
Provide empathetic, trauma-informed support, actively listening and guiding young people through emotional challenges.
Promote physical and mental wellbeing.
Use positive behaviour support strategies and reflective practice to manage challenging behaviours.
Build trusting, respectful relationships and empower young people to make informed decisions.
Promote independent living skills including budgeting, cooking, personal hygiene, job readiness, and communication.
Facilitate participation in community activities, appointments, and social events.
Work collaboratively with colleagues, managers, and external professionals to deliver holistic support.
Engage in reflective practice, supervision, and ongoing professional development.
Lead and promote an open, transparent, and reflective culture within the workplace.
Complete all tasks and additional duties requested by management within the allocated timeframe.
Adhere to all organisational policies, procedures, and regulatory standards, including safeguarding, health and safety, equality, and diversity.
Maintain awareness of relevant regulations, safeguarding procedures, and best practices in high-needs support.
Support audits, inspections, and quality assurance processes.
Ability to work independently and collaboratively within a team.
Calm under pressure and committed to safeguarding and young peoples welfare.
