This full-time position is ideal for an experienced healthcare leader with a strong commitment to patient safety, high-quality clinical care, fiscal accountability, and system-level patient flow, and a passion for improving transitions from hospital to community.Reporting to the Regional Director, the Area Director is accountable for the overall clinical, operational, and financial performance of the Scarborough Transitional Care Unit (TCU). This includes ensuring the delivery of safe, evidence-based, patient-centered care, effective use of resources, and timely, well-coordinated patient transitions. The role requires a system-focused mindset, a collaborative leadership approach, and the ability to guide teams through complex clinical, operational, and regulatory environments.The successful candidate will work closely with hospital partners, retirement home operators, internal clinical and operational teams, funders, and regulatory bodies to ensure seamless transitions, consistent clinical standards, strong patient safety practices, and exceptional client experiences. This is a highly influential role for a leader who excels at clinical accountability, patient safety leadership, performance management, and integrated care delivery.What You’ll DoClinical Care & Patient Safety Accountability* Maintain overall accountability for patient safety and quality of care within the Scarborough TCU* Ensure clinical services are delivered in accordance with professional standards of practice, Bayshore policies, regulatory requirements, and contractual obligations* Provide leadership oversight to Clinical Managers / TCU Managers, ensuring safe staffing, clinical competency, and appropriate escalation of patient care concerns* Identify, mitigate, and manage clinical and patient safety risks, including adverse events, incidents, and near misses* Ensure timely investigation, documentation, follow-up, and corrective action related to patient safety events, complaints, and quality concerns* Promote a strong culture of safety, accountability, and continuous quality improvement across the TCUClinical & Operational Leadership* Provide direct leadership, supervision, and support to Clinical Managers / Transitional Care Unit Managers, ensuring high-quality care delivery and compliance with all standards of practice* Oversee day-to-day clinical and operational operations of the Scarborough TCU, ensuring safe, efficient care delivery and transitions for seniors* Serve as On-Call Manager and act as the TCU Manager when required to ensure uninterrupted clinical and operational oversight* Provide coaching, mentorship, and cross-training support to develop leadership capacity and ensure operational and clinical continuityPatient Flow & System Integration* Oversee and optimize patient flow across the TCU, from hospital referral through admission, length of stay, and discharge* Collaborate with hospital partners, care coordinators, and internal Patient Flow teams to ensure appropriate admissions, timely repatriation, and safe discharge planning* Monitor census, length of stay, clinical acuity, and discharge readiness, ensuring care plans align with patient needs and system capacity* Address patient flow barriers while maintaining clinical appropriateness and patient safety as primary decision driversFinancial & Performance Accountability* Maintain financial oversight of the TCU, including budget management, cost control, and responsible resource utilization* Balance fiscal accountability with safe staffing levels and quality care delivery* Monitor financial performance, census targets, and key operational metrics to ensure program sustainability* Identify financial and operational risks and work with the Regional Director and Finance partners to implement mitigation strategies* Ensure accurate documentation, billing, and compliance with funding, payer, and contractual requirementsPeople & Talent Management* Oversee recruitment, onboarding, training, and performance management for Clinical Managers, ensuring clinical competence and leadership readiness* Foster a culture of clinical excellence, accountability, engagement, and professional development* Support succession planning and workforce stability within the TCUQuality, Partnerships & Strategy* Monitor clinical quality indicators, patient safety metrics, operational KPIs, and client satisfaction, translating insights into action* Address patient, family, and partner concerns promptly, ensuring appropriate clinical review, documentation, and follow-up* Collaborate with hospital partners, funders, government programs, and regulatory bodies to support safe, integrated care delivery* Work closely with the Regional Director, Clinical Quality, and Integrated Care Solutions leadership to implement clinical policies, quality initiatives, and risk management strategies* Represent Bayshore ICS at community events, committees, and external meetings* Participate in business development, strategic planning, and market analysis, ensuring clinical safety and quality are embedded in growth initiatives* Contribute to the development and oversight of contractual agreements and shared account strategies, including clinical and patient safety expectations* Lead and participate in continuous improvement initiatives aligned with Bayshore’s Quality Management System ## Qualifications:What You Bring* Registered Nurse or other licensed healthcare professional, or an equivalent combination of healthcare leadership and facility-based operational experience* 10–15 years of experience in healthcare settings such as hospital, retirement, or long-term care* Demonstrated experience with clinical leadership, patient safety, and quality improvement* Proven ability to manage clinical risk, patient safety incidents, and quality concerns* Experience overseeing patient flow, discharge planning, or transitional care programs an asset* Strong understanding of the Canadian healthcare system, including OHIP, insurance, and private pay models* Knowledge of medical terminology, patient care best practices, and infection prevention and control* Strong relationship-building skills with internal teams, hospital partners, and external stakeholders* Excellent communication, judgment, and decision-making skills in complex clinical situations* Comfort with Microsoft Office, digital tools, and standard business technologies* A commitment to Bayshore #J-18808-Ljbffr
