About the Role We are seeking a dedicated and organised Care Coordinator to join our CQC-regulated Supported Living Service in Hemel Hempstead. This is an exciting opportunity for an experienced care professional who is passionate about delivering high-quality, person-centred support and ensuring compliance with CQC standards. As a Care Coordinator, you will play a key role in overseeing care delivery, supporting staff, and ensuring service users receive safe, effective, and compassionate care within supported living settings.
Key Responsibilities
Coordinate and manage care packages within supported living services Develop and review person-centred care plans and risk assessments Ensure compliance with CQC regulations and company policies Conduct staff supervisions, spot checks, and support visits Liaise with families, social workers, healthcare professionals, and external agencies Assist with rota management and staffing coordination Support with safeguarding concerns and incident reporting Maintain accurate and up-to-date care documentation Requirements
Previous experience in a Care Coordinator or senior care role Experience within Supported Living services Strong understanding of CQC regulations and compliance Excellent organisational and communication skills Ability to manage multiple priorities effectively Level 3 or above in Health & Social Care (preferred) Full UK driving licence (preferred) What We Offer
