We're seeking a highly organized and committed Care Coordinator for our St Helens branch. This is a key role in ensuring seamless care delivery and supporting both our team and Service Users.
Key Responsibilities:
Scheduling & Coordination Manage rotas to ensure Service Users receive consistent, quality care
Client Assessments Support assessments and risk evaluations to develop tailored care plans
Team Support Guide and mentor care staff to maintain excellent standards
Stakeholder Communication Work closely with Service Users, families, and healthcare professionals
Problem Solving Handle scheduling challenges, absences, and urgent changes effectively
On-Call Duties Provide out-of-hours support (1 in 4 weekends and 1 weekday night)

What You'll Need:
