At DHU Healthcare CIC , our Primary Care teams work at the heart of local communities, delivering high-quality, patient-centred services across the areas we serve. Based at Eyam Surgery , with flexibility to support Bradwell Surgery when required, you'll play a vital role in ensuring patients receive timely access to the right care and a positive experience from the moment they contact or visit the practice.
Our teams of skilled clinicians and support professionals work together to provide responsive, professional NHS services. As a Care Navigator, you'll be central to guiding patients to the most appropriate service, helping the practice run efficiently while always keeping patient wellbeing at the forefront. As a Care Navigator you will: Be the welcoming, professional first point of contact for patients and visitors, both face to face and on the telephone. Open and close Eyam and Bradwell Surgery as required, ensuring the premises are safe and ready for service.
