Were looking for a proactive, compassionate Carers Review & Support Officer to ensure Carers continue to receive the right support at the right time.

Youll be a key contact for Carers whose circumstances may be changing and your work will help strengthen families, protect wellbeing and make caring more sustainable.

Key Responsibilities As a Carers Review & Support Officer, youll: Contact all Carers who are due for their annual assessment review.

Ensure at least 95% of due reviews are completed each year.

Identify if a Carer may have substantial difficulty engaging in the review process (per the Care Act 2014) and arrange advocacy support when needed.

Help Carers explore whether their needs or caring responsibilities have changed since their last assessment.

Complete Review Forms where no changes have occurred to maintain ongoing support.

Carry out full reassessments where circumstances have changed, using National Eligibility Criteria under the Care Act 2014.

Co-produce care and support plans with eligible Carers, ensuring they reflect assessed needs and are practical and proportionate.

Submit support plans and personal budget recommendations to the Council and explain outcomes clearly to Carers.

Provide information, advice, signposting and preventative support for Carers who are not eligible under regulations.

Ensure all reviews and plans meet the Carers Assessment & Support Planning Quality Assurance Framework.

Use a whole-family approachrecognising other Carers, including young Carers, and making referrals with consent.

Keep Carers informed of their rights and available services.

Tailor support for Carers with a range of needs (e.g.

mental health conditions, learning disabilities, dementia).

Make outgoing referrals to help Carers access local services and community support.

Work collaboratively with other Review Officers to ensure a coordinated approach across the service.

Process notifications such as bereavements and changes in residential status sensitively and accurately.

Maintain high-quality case records on Local Authority and internal systems.

Produce accurate written reports to support service reporting.

Meet performance indicators set by your Team Leader.

Promote and safeguard the welfare of adults and children.

Contribute content to newsletters for Carers and professionals.

Assist as needed in the role of Assessment & Support Officer and help promote the wider service.

Signpost and support Carers who want to build or develop their own support groups and activities.

Help maintain a living directory of wraparound, local signposting resources.

Support the induction and development of social work students and volunteers.
