- You will required to work flexibly as part of the CWD short breaks team with an allocated caseload to assess the eligibility and need for a short break, secure the right provision to meet the need and ensure this is reviewed in line with regulatory requirements so that there is assurance that the short breaks plan is meeting the identified needs for the child and their family.
- You will need to have experience of working with children with disabilities and their families. You will bring substantial assessment experience (e.g., Early Help Assessments) and demonstrate excellent communication skills. You will work restoratively with children and their families and report to the Team Leader. You will work collaboratively with partners across education, health and care, focusing on outcomes for children.
