This role is crucial in designing and implementing pathway plans that promote young people’s move into independence, ensuring a smooth transition into adulthood. Day-to-day of the role: Undertake a comprehensive Social Work service to Children and Families within the leaving care service. Formulate and design appropriate pathway plans to meet identified needs and arrange the implementation of such plans with an emphasis on promoting independence. Provide ongoing support to individuals and families as the key worker, including home visits, meetings, direct work/observations, and completion of reports and assessments related to the child’s pathway plan.
