Pathways Social Worker Annual Salary: £32,654 - £42,708 plus a 15% market supplement paid monthly

Join our dedicated team as a Pathways Social Worker, where you will support young people aged 16-25 transitioning from care to independence. This role is crucial in designing and implementing pathway plans that promote young people’s move into independence, ensuring a smooth transition into adulthood.
Day-to-day of the role: Undertake a comprehensive Social Work service to Children and Families within the leaving care service.
Formulate and design appropriate pathway plans to meet identified needs and arrange the implementation of such plans with an emphasis on promoting independence.
Provide ongoing support to individuals and families as the key worker, including home visits, meetings, direct work/observations, and completion of reports and assessments related to the child’s pathway plan.
Ensure individual professional standards are maintained in line with agreed levels, including full compliance with policy and procedural guidelines.
Regularly feedback to the Team Manager on any issues related to the provision of services or policies.
Required Skills & Qualifications: A recognised Social Work qualification, e.g., CQSW, CSS, Dip SW.
