Youll be working closely with integrated discharge hub staff to support customers to be discharged from hospital and those who need support to stay safe and well in their own homes in the community. Please note We will reimburse mileage and parking expenses for any travel undertaken as part of your role. Job requirements Youll complete tasks to help customer get home safely from hospital such as doing property inspections, collecting belonging, dropping off food parcels, or arranging quotes for deep cleans so discharge can go ahead. Youll carryout a holistic assessment with customers and using an outcome star to complete an action plan to ensure you support the customer to help them stay happy, safe and well in their home.
