Introduction to the Department The Acute Medical Unit is a 39‑bed assessment and short‑stay ward located on the ground floor of the hospital. Of the 39 beds, 11 are side rooms (including 4 negative‑pressure isolation rooms for infection control purposes). Job Summary The care of patients is divided between two acute medical teams, each led by an acute medicine consultant, caring for 15‑16 patients. The department has developed SDEC services to prevent avoidable admissions and manage patients through ambulatory / same‑day emergency care pathways. Patients are referred to SDEC following emergency presentation to the ED, or directly from GP/NHS111 via a dedicated acute medicine Hot Phone. LAS conveys suitable patients to SDEC under the Trusted Assessor model. Patients also attend SDEC as part of early supported discharges from the AMU and the medical take. SDEC operates 8 am‑8 pm, 7 days a week, and the team is being expanded to meet increasing demand. Admission avoidance and early discharge are supported by a dedicated therapies team and specialty in‑reach from medical specialties. Main Duties and Responsibilities
Participate in clinical governance activities including incident reporting and patient safety initiatives. Provide senior clinical leadership and expert management for patients presenting with acute medical conditions. Contribute to safe discharge planning in partnership with community and primary care services. Promote early senior decision‑making to support effective patient flow. Provide clinical supervision, training, and mentoring to resident medical staff and support their professional development. Show flexibility and undertake different appropriate clinical tasks at the request of the clinical manager, as the need arises. Provide clinical supervision for specialist registrars.
Shift Patterns and Workload For a whole‑time 10 PA contract the baseline split is: Direct Clinical Care: 8.0 PAs per week; Supporting Professional Activities: 1.5 core SPAs and 0.5 quality/service SPAs (subject to agreement). An additional 0.5 SPA is available to all Consultant staff, subject to review and agreement. Job plans are reviewed annually.
There are different shifts within the annualised Acute Medicine Consultant rota: - AMU A & B: 08:00‑16:00 (1.75 DCC PAs + 0.25 SPA per day), 8‑10 am post‑take round in ED, named consultant for half of AMU. - SDEC: 08:00‑17:00 (2.0 DCC PAs + 0.25 SPA per day), named consultant supporting ANPs and resident doctors. - PTWR & Day Take: 08:00‑17:00 (2.25 DCC PAs), post‑take round in ED, named consultant for the medical take. - Weekday Evening Take: 17:00‑20:00 (0.83 DCC PAs). Frequency: 1 shift per month plus an occasional Friday shift. - AMU Weekend: Alternate Saturday 08:00‑20:00 (4 DCC PAs) / Sunday 08:00‑17:00 (3 DCC PAs) with a reverse pattern on alternate weekends.
Dedicated time for DCC admin duties (reviewing results, attending MDT meetings, dictating letters, virtual clinics, etc.) is included in the AMU and SDEC shifts (0.25 PA per shift). There is evening and weekend scheduled working as per the above shifts. The post does not currently include overnight or weekend out‑of‑hours on‑call rota; no on‑call supplement applies. Person Specification Essential Qualifications, Training and Experience
MRCP or equivalent. CCST/CCT or equivalent in Acute Medicine / General Internal Medicine and on the specialist register, or otherwise within 6 months of CCT date at interview, or CESR application submitted. Experience of working in acute medicine. Ability to take full and independent responsibility for clinical care of patients. Ability to take full and independent responsibility for the management of acute emergencies.
Desirable Qualifications, Training and Experience
Post‑graduate degree. Experience in Ambulatory Emergency Care / Same‑Day Emergency Care.
Essential Skills & Abilities
Ability to communicate effectively with staff, patients, and relatives. Potential to develop leadership skills. Excellent interpersonal and organisational skills. Experience of service development and quality improvement initiatives. Ability to work in a pressurised environment.
Desirable Skills & Abilities

Essential Knowledge
Knowledge and participation of clinical audit. Understanding of Clinical Governance. Experience in providing high‑quality training and education to undergraduates and postgraduates.

Other Requirements
Excellent communication skills, both oral and written, and an ability to relate to staff and patients. Knowledge of resource management issues. Ability to stay calm under pressure. Computer literate. Willingness to work flexibly as part of a team. GMC Registration.

Ability to support and motivate staff during periods of pressure.
