Were looking for a passionate and skilled Social Worker to join our Transitions Service, supporting young people aged 1725 as they move from childrens services into adult social care. This is a rewarding opportunity to make a real difference at a crucial stage in their lives. About the role As a Social Worker in our Transitions team, you will: Carry out strengths-based, person-centred Care Act assessments. Be the named worker for young people transferring into adult services. Build strong, trusting relationships with young people and their families while working alongside childrens social care. Develop support plans that promote independence and ensure eligible needs are met. Review plans regularly, ensuring least-restrictive, outcome-focused practice. Work collaboratively with professionals across directorates, partners and external agencies. Identify and manage risks, safeguarding young people and vulnerable adults. Keep accurate, timely records on our social care systems. Provide guidance and informal supervision to colleagues and contribute to the development of the wider team. This role plays a key part in supporting young people to achieve their potential and enter adulthood with the right care in place.
