Job Title: Family Support Hub Coordinator Location: Spectrum Centre, 331-333 Shankill Road, Belfast BT13 3AA Contract : 31 March 2026, extension subject to funding. Hours: 20 hours per week Salary: £26,475 (pro rata) Annual Leave: 25 days per annum plus statutory entitlement (pro rata) JOB SUMMARY / MAIN PURPOSE The Children & Young Peoples Strategic Partnership (CYPSP) is a regional framework designed to promote integrated planning and service delivery to improve outcomes for children, young people and their families. As part of this, a network of Family Support Hubs have been established across Northern Ireland. The Hubs are intended to provide a structure through which early intervention services can be delivered more effectively, by matching local needs to available services, and encouraging those providing services to work more closely with each other within a supportive network. The Family Support Hub Co-ordinator is responsible for building productive working relationships with relevant voluntary, statutory and community organizations, facilitating meetings of the Hub, and ensuring that the robust systems are in place to manage the referral process in terms of safeguarding, data management and confidentiality. KEY TASKS Service Request & Delivery To provide an open and transparent co-ordination service for Family Support Hub activities. To act as a central point of contact for Hub referrals. Screen referrals for appropriateness for hub services ensuring that families have given informed consent. Where required, contact families to carry out risk assessment and assessment of need. Carry a small Family Support caseload. Identify potential service options (matching family need to appropriate service to address this need) including a search on Family Support NI Database. Process referrals where appropriate (i.e. moving from service request to service delivery). Processing referrals following meetings, making sure referrals are passed to the appropriate service. Communicating with referring agencies in relation to the following: Identification of service(s) to meet need Uptake of and engagement in services Notification of conclusion of service Meetings Prepare papers / information for Family Support Hub meetings in a timely manner including: Agenda and Minutes of previous meeting, Confirmation of action on referrals since previous meeting, Summary of new referrals requiring consideration (headline data only), Information on outcomes of referrals previously referred from the Hub Attendance at Hub Lead body & Regional Collective Hub meetings Networks Create and maintain a collaborative network of providers for the Family Support Hub and provide information to demonstrate the effectiveness of the network. Support and encourage partners to develop services to meet emerging needs, including partnership working towards programme development Identify and provide information to demonstrate the level of unmet need for early intervention family support services. Family Support To manage a case load, and undertake visits to home, school and other community settings as appropriate. To work with children and families, mainly in the home setting, to enhance the childs development and the parents understanding of their own crucial role in this including parent/child interaction. To carry out a robust holistic assessment of needs alongside the family. To develop and implement an action plan for intervention and support for each family. Administration To ensure implementation of appropriate reporting and data collection mechanisms To assist in timely delivery of all reports and administration To assist with the maintaining of accurate and up to date records To ensure that all necessary information is recorded and stored appropriately with respect to confidentiality Maintaining Hub monitoring and evaluation records as required by Belfast Health and Social Care Trust/ Outcomes Group / CYPSP and in compliance with GDPR. Develop and maintain data to demonstrate that access to early intervention family support services is improved by matching the needs of referred families to family support providers and that awareness of local family support services has increased. Management & dissemination of resources to families, for example: vouchers, Christmas supports, ASD resources. Training To attend any relevant training General To contribute as part of a team to the overall delivery of Greater Shankill Partnership Programmes within the Greater Shankill area. To carry out other duties appropriate to the grade and post as assigned by the line manager. Personnel Specification Essential Criteria NVQ level 3 or above in a relevant discipline e.g., Criminal Justice, Youth/Community Work, Social Work, Health, Child Care or Education At least two years experience working in a similar post with children and families Experience of working in partnership with parents, children and professionals Knowledge of issues affecting families from a disadvantaged background Experience of working with children and families within the family home or in a community based setting. Understanding of and ability to apply Adult Safeguarding and Child Protection procedures Ability to act as an advocate for children, young people and families Excellent organisational and time management skills including planning and prioritisation of work. Excellent IT skills ability to use ICT packages for recording and reporting Demonstrable evidence of good oral and written communication skill Ability to work on own initiative, be self-motivated, flexible and work under pressure. Desirable Criteria Knowledge of the issues facing the Greater Shankill area of Belfast
