We are seeking a highly motivated and values-driven Occupational Therapist to join our newly established Care at Home Review Team. This is an exciting opportunity to shape a dynamic new service that ensures care at home packages remain person-centred, proportionate, and aligned with individual outcomes. As part of the Care at Home Review Team you will be working across both urban and rural communities. As an Occupational Therapist, you will bring your expertise in functional assessment, and environmental adaptation to the review of care at home packages. Working across both urban and rural communities, you will: Undertake outcome focused reviews of care packages, ensuring they remain responsive to changing needs. Apply your professional skills to assess functional ability, independence, and participation in daily life. Identify opportunities for rehabilitation, reablement, and equipment provision that reduce reliance on formal care. Collaborate closely with service users, carers, and multi-agency partners to promote wellbeing, safety, and independence.
