Outreach Caseworker Location: Newcastle Salary: £31,262 per annum Hours: 35 per week Contract: Fixed term contract (April 2026 - March 2027) with strong possibility of an extension for a further two years
Additional Requirement: Applicants must have a full licence (2 years) and access to a vehicle.
Are you ready to make a real difference by helping people reduce their reliance on emergency care services? Do you have expertise around smoking cessation? If so, this is a fantastic opportunity to be part of a pilot project within our High Intensity Use team.
Do you want to help people navigate complex health and social care systems? Are you passionate about reducing unnecessary emergency service use and improving lives? As an Outreach Caseworker, you'll work directly with individuals who frequently access urgent care services. You'll help them identify unmet needs, build coping strategies, and connect with the right support. This role is vital in creating a more compassionate, efficient, and person-centred care environment.
What will a day in the life of an Outreach Caseworker involve? Supporting individuals with complex needs and/or smoking related respiratory complaints to reduce their use of A&E, 999, and mental health services.
