Location: East Sussex covering areas which include Rother, Hastings, St Leonards, Battle, Bexhill, Sidley, Rye, Northiam
Salary: £30,000 per annum, pro rata
Hours: 35 per week
Contract: Fixed term contract (Maternity cover to October 2026)

Additional Requirement: Applicants must have a full licence (2 years) and access to a vehicle. Or be eligible for Access to Work Scheme.

As an Outreach Caseworker , you'll work directly with individuals who frequently access urgent care services. You'll help them identify unmet needs, build coping strategies, and connect with the right support. This role is vital in creating a more compassionate, efficient, and person-centred care environment.

What will a day in the life of an Outreach Caseworker involve? Supporting individuals with complex needs to reduce their use of A&E, 999, and mental health services.

Building strong relationships with health, social care, and emergency service partners.
