Role Description

This is a full-time on-site role for a Senior Clinical Pharmacist located in London. The Senior Clinical Pharmacist will play a critical role in supporting PCNs by delivering direct, high-quality pharmaceutical care and enhancing medication safety. Responsibilities include providing expert clinical advice to healthcare professionals, managing and reviewing patient medications for safety and efficacy, conducting medication reconciliation, and leading projects to optimize medicine management. The role involves patient consultations, supporting chronic disease management, supervising pharmacy teams, and ensuring compliance with relevant clinical guidelines and standards.


Qualifications

  • Strong knowledge and skills in Pharmacy Practice and Pharmacy operations
  • Expertise in Pharmaceutical Care and comprehensive medicine use evaluation
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Proficiency in medicine-related tasks such as prescribing, review, and patient-centered care
  • Able to plan, manage, monitor, advise and review general pharmaceutical care programmes for patients across core areas, including disease states/long term conditions
  • Effective communication and collaboration skills to work with multi-disciplinary teams
  • Demonstrates ability to identify and resolve risk management issues according to site policies and protocols
  • Demonstrates ability to conduct teaching and assessment effectively, ensuring there is appropriate clinical supervision and support in place for all pharmacists and pharmacy technicians
  • Experience working in general practice, and understands the pharmacy role in governance across primary care
  • Completion of an accredited pharmacy degree and registration with a recognised Pharmacy regulatory body i.e. General Pharmaceutical Council (GPhC)
  • Independent prescriber, or working towards/intent of gaining an independent prescribing qualification
