We are seeking a dedicated and compassionate Practice Nurse to join our friendly and supportive primary care team. Ideally 25-30 hours per week The successful candidate will be a highly motivated and proactive Practice Nurse who brings a positive, can-do attitude to their work and thrives in a collaborative team environment. You will actively contribute ideas, support service improvements, and help drive forward high standards within the practice. In this role, you will work closely with GPs, Practice Nurses, Nurse Associates, Healthcare Assistants and administrative staff, building strong working relationships to ensure seamless, coordinated care. You will also play an important part in mentoring, sharing knowledge, and supporting the development of others within the team. Key Responsibilities Clinical Practice Deliver high-quality nursing care within the practice. Conduct patient consultations (face-to-face and telephone). Assess, plan, implement and evaluate individual care plans.
Chronic Disease Management Lead and support clinics for diabetes, asthma, COPD, and cardiovascular disease. Monitor key indicators such as HbA1c, blood pressure, and cholesterol. Provide lifestyle advice and patient education. Support insulin initiation and ongoing management. Support patients with smoking cessation. Contribute to the management of other long-term conditions.
Immunisations & Vaccinations Administer childhood immunisations in line with national guidelines. Provide travel health advice and vaccinations. Deliver seasonal vaccination programmes (e.g. flu, RSV, Pneumococcal). Maintain vaccine stock and cold chain integrity.
Health Promotion & Screening Deliver cervical screening (smear tests). Support national screening programmes. Promote healthy lifestyle choices.
Treatment Room Duties Wound care, dressings, and suture removal. Ear irrigation and venepuncture. Process pathology tests and results.
Patient Education & Support Encourage self-management and independence. Provide structured education to patients and carers.
Professional & Administrative Duties Maintain accurate patient records (EMIS, Docman). Participate in audits and quality improvement. Engage in ongoing CPD and training. Work collaboratively with GPs and the wider healthcare team.
Person Specification Essential: Registered General Nurse with valid NMC registration. Minimum 5 years post-qualification experience. Qualified cervical smear taker. Diploma/BSc level qualifications in diabetes, asthma, or COPD. Immunisation and vaccination trained. Experience using EMIS, Docman, and iPlato (desirable). Strong understanding of QOF and safeguarding. Excellent communication and interpersonal skills. Ability to work independently and as part of a team.
Desirable: At least 2 years experience in primary care. Independent prescribing qualification (for chronic disease management). Full UK driving licence.
