We are looking to recruit a Qualified Social Worker in the Hospital Discharge Team within Hackney. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.

The responsibilities of the Social Worker includes:

Assess the social care needs of adults in an acute hospital setting to support timely, safe discharge Complete Care Act assessments, mental capacity assessments, and best interest decisions in line with legislation Work collaboratively with health professionals, families, and partner agencies to coordinate discharge planning Arrange appropriate packages of care, placements, and community support services Safeguard adults at risk and respond to concerns in line with statutory duties and local procedures Contribute to multidisciplinary team meetings and discharge planning forums Maintain accurate, timely, and legally compliant case records Promote strengths-based, person-centred practice and support service user choice and independence Manage a complex caseload, prioritising effectively to meet service demands and discharge targets. The ideal candidate will have:

Social Work England registration Eligibility to work in the UK Social Work qualified (MA/BSc/DipSW/CQSW/MSc/BA) Working with Ackerman Pierce you will be guaranteed weekly payment and access to continued profession development, including a dedicated consultant to support you through the interview and on boarding process.
