Develop and implement care plans for clients with complex medical needs, in collaboration with the multidisciplinary team. Monitor and evaluate the effectiveness of care plans, making necessary adjustments to ensure the best possible outcomes for our clients. Manage and train a team of care staff, providing guidance and support to ensure they deliver care in line with company standards and policies. Maintain accurate records and documentation in accordance with regulatory requirements.
