Overview

12 month fixed term contract for up to 6 sessions, salaried GP. Post holder will manage a caseload in primary care ensuring highest standards of care.

Responsibilities

  • Make yourself available for surgery consultations, telephone/video/e‑consultations, home visits, prescription signings and timely handling of queries, paperwork, test results, and correspondence.
  • Make professional, autonomous decisions regarding presenting problems, whether self‑referrals or referrals from other health care workers.
  • Assess health care needs of patients with undifferentiated or undiagnosed problems.
  • Screen patients for disease risk factors and early signs of illness.
  • In consultation with patients and following current practice disease‑management protocols, develop care plans.
  • Provide counselling and health education.
  • Admit or discharge patients to/from the caseload, and refer to other care providers as appropriate, including providing in‑patient care for specific patients in the community hospital.
  • Record clear and contemporaneous consultation notes to agreed standards.
  • Collect data for audit purposes.
  • Compile and issue computer‑generated acute and repeat prescriptions, avoiding handwritten prescriptions whenever possible.
  • Prescribe in accordance with the practice prescribing formulary.
  • Lead the practice in particular areas of clinical function such as prescribing.
  • Maintain awareness and compliance with practice policies and guidelines (prescribing, confidentiality, data protection, health & safety).
  • Commit to lifelong learning and audit to ensure evidence‑based best practice.
  • Contribute to evaluation/audit and clinical standard setting within the organisation.
  • Contribute to the development and summarising of computer‑based patient records and Read‑coding patient data.
  • Attend training and events organised by the practice or other agencies.
  • Supervise GP registrars and medical students within the practice and other training activities as agreed with the trainer.
