Senior Community Learning Disabilities Nurse

An exciting opportunity has arisen within the Ealing Community Team for People with Learning Disabilities within the Nursing team. The CTPLD is looking for a part time band 6 Learning Disabilities Nurse to join the team.

The post is a part time post, of 18.5 hours per week / 0.5FTE. Flexibility can be offered as to working days with the option of some remote working.

The nursing team is a strong, passionate and dedicated team of RNLD's who endeavour to improve the health outcomes for people with learning disabilities across Ealing.

The successful candidate will be responsible for implementing the nursing model to assess, plan implement and evaluate care, ensuring people's physical health needs are met; to support mainstream health services to meet the needs of people with learning disabilities and to support hospital admission and discharges.

The successful candidate will have the opportunity to lead on service development projects and implement innovative nursing and multi-disciplinary pathways. You will work alongside the multi-disciplinary team including psychiatry, nursing, psychology, PBS, speech and language therapy, occupational therapy, physiotherapy to ensure a holistic approach is taken to manage health needs.

Main duties of the job

Qualifications and Training

• Registered Nurse - Learning Disabilities
• Experience of working with people with a learning disability
• Further education relating to common physical health conditions associated with people with a learning disability.

Knowledge and Experience

• Relevant experience in working as a learning disabilities nurse
• Good understanding of the needs of people with learning disabilities.
• Experience of managing the physical health needs of people with learning disabilities.
• Basic understanding of mental health needs of people with learning disabilities
• Knowledge and understanding of health care systems and structures
• Experience of working in a community setting
• LeDeR training
• Experience of safeguarding
• Experience of facilitating training
• Experience of working with the Mental Capacity Act

• Supervision of junior members of staff
• Previously supported student nurses on placements within an LD nursing setting
