Your Role And Responsibilities
As a Senior Independence and Wellbeing Practitioner, you’ll play a vital role in reviewing the care and support needs of people receiving services. You’ll ensure that support plans are person-centred, outcomes-focused, and of the highest quality.
On a typical day, you might:
- Carry out assessments under the Care Act 2014 and review existing care packages.
- Evaluate the quality and sustainability of commissioned services.
- Assess mental capacity under the Mental Capacity Act 2005.
- Complete Continuing Health Care checklists.
- Conduct phone assessments, home visits, and attend professional meetings.
- Work independently in the community, supported by a comprehensive induction and ongoing training.
You will need

- A good understanding of safeguarding, the Care Act, and the Mental Capacity Act.

Please note: This role requires a full UK driving licence, access to a vehicle, and the ability to travel across county.
