Your Role And Responsibilities

As a Senior Independence and Wellbeing Practitioner, you’ll play a vital role in reviewing the care and support needs of people receiving services. You’ll ensure that support plans are person-centred, outcomes-focused, and of the highest quality.

The team

This is an exciting time to join our Learning Disabilities Review and Quality Assurance Team. We’re investing in new roles and ways of working to transform outcomes for people across Suffolk.

You’ll be part of a supportive, forward-thinking team that values co-production, professional development, and integrated working with health and social care partners. Together, we’re shaping best practice and creating new pathways that put people at the heart of care.

Please note: This role requires a full UK driving licence, access to a vehicle, and the ability to travel across county.
