Weare currently seeking Support Workers (Band 34) with experience in the following areas:
Adult Disability
Supported Living
Autism Outreach
Community Living
Autism Hub
Responsibilities
Supporting individuals with daily living activities
Encouraging independence and community participation
Delivering person-centred care in line with individual support plans
Working collaboratively with multidisciplinary teams
Maintaining accurate records and following safeguarding procedures

Requirements
Applicants must have the right to live and work in the UK.
NVQ Level 3 in Health & Social Care (or equivalent)
A compassionate, reliable, and professional approach to care
Strong communication and teamwork skills
