Job Summary:

The postholder will practice as an Urgent Care Practitioner and support the daily clinical team to provide an on-the-day urgent care service for patients.

The role will involve the assessment, diagnosis, treatment or referral of patients presenting with undifferentiated and undiagnosed conditions.

The post-holder will work within the Urgent Care Practitioner/Advanced Practitioner remit within their level of competency.

Main duties and responsibilities:

1) Clinical

To provide high quality patient care

To provide assessment, diagnosis and treatment at first point of contact by attending to patients according to patients needs generally within the practice setting.

To support and continue the development of Urgent Care services in Emerald Health.

To accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.

To evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral.

To undertake and interpret diagnostics and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis. To use broad clinical guidelines (NICE etc) relating them to individual cases, providing assistance and feedback to the organisation and others on how these should be interpreted.

To signpost appropriately to other services in the identified healthcare pathway.

To refer patients to appropriate care settings, within locally agreed healthcare pathways.

To discharge or treat, as appropriate and within the scope of practice.

To maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.

To advise and support members of the public on healthy lifestyles, health promotion and disease prevention.

To provide clinical advice to other members of the team where appropriate.

The UCP would be expected to act within their competencies and seek advice from other team members when necessary.

Ensure that the care given meets the requirements of local and national guidelines, policies and standards.

Respond to medical emergencies, incidents and accidents as competent to do so.

Trained in Immediate Life Support.

To ensure that all daily records for fridge temperature, key safe log, emergency drug box and cupboard are checked within the correct timescales and any anomalies are reported to the local manager.

Maintain Cold Chain procedures

Provide information, advice (within scope of practice) and support to patients face to face and via the telephone.

Promote & participate in maintaining a safe comfortable & therapeutic environment.

Involved in team discussions to ensure best practice

To participate in outreach clinics providing healthcare and lifestyle advice to hard-to-reach groups

To work with the ethos of Emerald Healthcare to ensure a safe, professional and enjoyable workplace.

Use appropriate infection control procedures and maintain work areas in each clinical room so that they are clean, safe and free from hazards reporting of any potential risks identified, including:

1. hand washing

2. universal hygiene procedures

3. collection and handling of laboratory specimens

4. segregation and disposal of waste materials

5. decontamination of instruments and clinical equipment

6. reporting and treatment of sharps injuries

7. dealing with blood and body fluid spillages

8. Assist patients and colleagues in adopting sound infection control measures

2) Administration

To maintain accurate records and data on Treatment Room activities in a timely fashion using the relevant clinical system.

To work as an effective team member and promote a multi-agency collaborative approach to health care.

Maintain the established reporting documentation and communication system to ensure efficient evaluation and monitoring.

Report and record all clinical incidents to local Manager.

Maintain and ensure the effective use of clinical stock levels

3) Communication

Establish and maintain effective working relationships with multi-professional teams across organisational boundaries, including primary care, social services and secondary care providers according to patient needs and appropriate care pathways.

To maintain communication with all healthcare professionals and outside agencies to ensure seamless, patient-centred service provision.

To ensure effective communication so that the patient receives the appropriate level of care and wherever possible avoids unnecessary hospital admission.

To take an active role in maintaining expert communication with all patients, carers and relatives which meets their individual needs.

To communicate effectively using a variety of communications skills: verbal, & non-verbal, written and electronic, with a range of individuals and organisations.

Make all reasonable attempts to overcome any barriers to understanding that may occur, giving due consideration to ethnicity, language, educational ability, barriers caused by illness/distress etc. which may adversely affect communication.

Assess individual communication needs and understanding for informed consent.

To deal with complaints in a calm and courteous manner, ensuring that, wherever possible, complaints are dealt with efficiently and satisfactorily on a local level.

To communicate effectively in demanding and challenging situations, where there may also be hostility

To assist in maintaining accurate data collection.

4) Education, Training & Research

To take responsibility for own professional development undertaking training for specific clinical tasks

Participate in research programmes as appropriate.

Participate in the development and implementation of standards and audit activities.

Attend all mandatory training courses and the updates and any study days as appropriate to the post

Attend performance and development reviews with your line manager.

Professional

To input to and extract information from the clinical IT system.

To work collaboratively with other members of the healthcare team such as the District Nursing Team, Mental Health Services, staff in Care Homes, Ambulance Service, Secondary Care etc.

To ensure that the wellbeing of patients and staff is maintained in accordance with local policies.

To undertake clinical audits in line with the organisations clinical audit plan, and to participate as required in equipment testing and other research.

To contribute to the analysis of information to inform future development of the service.

To participate and contribute to clinical meetings.

To identify risks to service provision, and highlight them to the Clinical Lead.

To work within the requirements of Data Protection legislation as it applies to patient records.

To maintain current professional registration.

To act at all times in such a manner as to promote confidence and public trust and to uphold the reputation of Emerald Healthcare and their own profession.

Managerial

To ensure effective time management and prioritisation of workload.

To ensure the security, safe storage and maintenance of any allocated equipment, medications and goods.

To ensure practice is within good clinical governance and based on sound up-to- date clinical evidence. Where appropriate, to identify and assess any risks associated with patient care or to oneself and communicate these risks to the appropriate clinical manager.
