Job plan: Provide two sessions to support frailty hospital at home and Urgent community response MDT and One team MDT: 2 PA. Provide 2 sessions offering comprehensive geriatric assessment to patients living in their own home: 2 PA. Provide one clinic per week focused on frail patients: 1 PA. A session for clinical administration: 1 PA. Educational and/or clinical supervision for a 4 month rotational FY2 post in community geriatrics and SpR: 0.5 SPA. SPA for appraisal and revalidation: 1.5 SPA.

The applicant will be expected to provide community-based care and work within a strong multidisciplinary team. Strong communication, clinical and judgment skills are expected, to contribute to these services and develop related interests.
